Fatty Liver Disease: Why a Liver Detox Won't Fix It
If you're reading this, there's a good chance it's because someone, probably your GP, mentioned fatty liver after a scan or a blood test, and then didn't tell you much more than that.
Maybe you were told to lose some weight.
Maybe you were told your liver enzymes were "a bit elevated" and to come back in six months.
Maybe you've already started googling liver detoxes, or you've picked up a bottle of milk thistle from the health shop and you're hoping that will help.
Many of my clients approached me for the first time, for this very reason. This is one of those conditions where what most people think they need to do and what actually works are two very different things.
What is fatty liver disease?
Fatty liver disease — formally called Non-Alcoholic Fatty Liver Disease, or NAFLD — means your liver is accumulating fat. Not because you've been drinking too much, but because of what's happening metabolically in your body.
A healthy liver stores a small amount of fat. That's normal. Fatty liver disease is diagnosed when more than 5–10% of the liver's weight is fat — and at that point, the organ responsible for over 500 metabolic functions starts to struggle.
Here's what most people aren't told: this condition is progressive.
It starts with fat accumulation — often with completely normal liver enzymes and no symptoms at all. It can then progress to inflammation, then scarring, then cirrhosis.
Each stage is harder to reverse than the last.
The good news — and this matters — is that in the early stages, fatty liver disease is one of the most reversible conditions in medicine. But only if you address what's actually causing it.
It's not really a liver problem. It's a metabolic one.
This is the part your GP probably didn't explain.
The strongest known predictor of fatty liver disease is insulin resistance. Having insulin resistance increases your risk fifteen-fold — regardless of your weight. Even lean women with insulin resistance develop fatty liver.
When your liver becomes insulin resistant, two things go wrong at the same time. First, the liver stops storing glucose properly and starts releasing it back into the blood — even when blood glucose is already high. This pushes insulin up further. Second, that elevated insulin tells the liver to convert glucose into fat and to hold on to it.
At the same time, if your fat cells — particularly the visceral fat stored around your organs — have also become insulin resistant, they start leaking fatty acids into the blood. The liver pulls those in and, because insulin is high, stores them too.
High insulin. High circulating fat. A liver being told to store everything and burn nothing. That's the metabolic storm that fills the liver with fat.
If this sounds familiar - if you're carrying weight around your middle that won't shift, if your triglycerides came back high on your last blood test, if you're fatigued and feel like you're eating reasonably and exercising without seeing change, this is very likely what's happening behind the scenes. These aren't separate problems. They're all part of the same metabolic picture.
Why a liver detox won't fix this.
I know the instinct. Your liver is struggling, so you want to support your liver. That makes sense.
But here's the problem: a liver detox doesn't address insulin resistance. And milk thistle — which does have genuine evidence for supporting liver cells — can't outwork a metabolic environment that is continuously telling your liver to make and store fat.
It's like mopping the floor while the tap is still running.
Supplements like milk thistle, omega-3 fatty acids, and vitamin E can support recovery — but they work best alongside the changes that actually turn off the tap.
On their own, they are not enough.
Can fatty liver be reversed?
The most effective intervention for fatty liver disease is not a supplement. It's not a medication — there is still no single effective drug for this condition, which is part of the reason your doctor may not have offered much guidance.
What reverses fatty liver disease is changing what arrives at your liver every day.
Reducing your carbohydrate intake lowers insulin. Lower insulin does two things — it slows down fat production in the liver, and it allows the liver to start burning the fat it has already stored. The liver is the most regenerative organ in the body. Give it the right metabolic environment, and it responds.
Cutting fructose — the sugar in fruit juice, sweetened drinks, and processed foods — is the single highest-priority dietary change. Fructose is metabolised almost exclusively by the liver, much like alcohol, and most of it is converted directly to fat.
Losing 7–10% of body weight resolves the inflammatory stage of fatty liver in roughly two thirds of cases. But even without significant weight loss, reducing insulin and removing fructose measurably reduce liver fat.
This isn't a general "eat healthy" recommendation. It's a specific, evidence-based approach that directly targets the metabolic mechanism driving the condition.
